Health Improvement Practitioner

​A health improvement practitioner is an extension to our team that provides help and support to patients with any problem they may have.

These are FREE Appointments for enrolled patients

Tui is our Health Improvement Practitioner (HIP) and she avaible on Tuesdays, Wednesdays and Fridays for appointments. Your doctor or practice nurse can introduce you to Tui or you can ask for an appointment at reception.

Appointments can be in the practice or via phone or video call.

HIP visits are approx. 30 minutes in length

Community Support Worker

A community based walk alongside support that can help you with:

  • Physical health or healthy lifestyle / Emotional health & wellbeing / Housing / Social engagement
  • Paid work / WINZ / Money matters / Family/whānau issues / Managing drug use, drinking or gambling Managing medications and medication planning / Loneliness

These are FREE Appointments for enrolled patients

Kogie is our Community Support Worker (CSW)

Your doctor or practice nurse can introduce you to Kogie or you can ask for an appointment at reception.

 

Health Coach

Our health Coach can help with gaining the knowledge, skills and confidence to help you reach your health goals.

Tips for managing and living well with long term conditions like:

  • Diabetes
  • Asthma
  • Gout
  • High cholesterol
  • Low blood pressure

Different ways to change your eating habits, get more active and meet your weight goals.

Help understanding your medication.

Also:

  • Positive parenting
  • Reducing or quitting smoking
  • Connecting you with the right social service, community group or charity to help you with housing, food or social connection.
