Easily connect with us using the Tō Mai patient portal! You can use Tō Mai to access your health records, book appointments, request prescriptions, and more.

Download the Tō Mai app or access the portal online using the links below.

The first time you access Tō Mai, you will need to enter your details and register with the Invite Code emailed to you.

Note: to successfully register, your details must match what we hold on file.

Only enrolled patients 16 years and over can access the Tō Mai App

Tō Mai makes looking after your health simple. You can:

Manage My Health Closure

Courtenay Medical will no longer be using Manage My Health, we have moved to the Tō Mai portal.

No data is transferred from MMH to the new patient portal.

If you wish to have your stored data deleted from MMH this will happen when you close your account with them.

How to Close Your Manage My Health Account

  1. Sign in to your Manage My Health account — either in the app or via the web portal.
  2. Go to your Profile / My Account section.
  3. Select “Close Account” from the menu.

After you confirm the closure, your portal access will end and stored health records in the portal will be deleted and become permanently inaccessible.

Important: Once closed, you cannot reopen the same account — you would need to register again if you wanted access later.

What Happens to Your Data

  • Your health records stored in the portal are deleted.
  • Some basic account identifiers (like email and verified identity level) may be retained for audit or legal purposes depending on the system’s data retention policies.

Need Help or Can’t Close It Yourself?
If you have trouble closing the account through the app/website, you can:
